TAE BO® CERTIFICATION WAIVER 
(Liability/Publicity/Release)

I am 18 years of age or older. I wish to participate in Tae Bo® Certification Course (the Event) a ZOOM or IN PERSON Class. I am voluntarily participating in the Event, and I expressly assume any and all risk of injury associated (directly or indirectly) with my participation in the Event. 

Prior to participating in the Event, I have consulted with a physician, to the extent that (in my sole discretion) I feel is necessary, and am satisfied that, to the best of my knowledge and belief, I am fit to participate in the Event and that I do not suffer from any medical condition, disability or ailment which would result in injury to me (directly or indirectly) from participating at the Event.

I hereby, for myself, my heirs, executors and administrators release, discharge and agree to hold harmless Billy Blanks®, Tae Bo® Nation, Sports Etc. (d/b/a Sports Profiles Plus, SPP Sports, Celebrity Sweat), and their respective parent, subsidiary and affiliated companies, and all of their respective directors, officers, shareholders, employees and agents from and against any and all injuries, claims, demands, losses or liabilities of any nature whatsoever which I may now or hereafter be entitled to assert in conjunction with my participation in the Event. The Released Entities have not arranged for nor carry any insurance of any kind for my benefit or that of my heirs, executors and administrators relative to my participation in the Event; and obtaining any such insurance is my sole responsibility.

I hereby transfer, convey and assign to the Released Entities in perpetuity and worldwide any and all right, title, and interest, that I have or may have, in and to any photograph(s) and/or video(s) taken of me during the Event (including all derivative and moral rights) as well as any and all right, title and interest in and to the copyright(s) in such photograph(s) and/or video(s).

I further agree, as between myself and the Released Entities, that the photo(s) and/or video(s) in any form, format, or media now known or hereafter developed, including without limitation audio, video, written, printed, graphic, electronic or digital, shall be wholly and fully owned by the Released Entities, and that I shall make no claim of any right, title or interest therein to the photo(s) and/or video(s) or the copyright(s) in the photo(s) and/or video(s) or any other electronic or digital format. I further acknowledge that I shall have no right of review or approval of any use of the photo(s) and/or video(s) and agree that the photo(s) and/or video(s) may be used and published world-wide for purposes of advertising, trade, publicity, promotion or any other purpose to be determined by the Released Entities in their sole and absolute discretion.
